We are committed to providing you with the best care possible. Here are a few things that you can do to help us make your visit run smoothly:

We pre-register our patients prior to the visit in an effort to keep wait times down and to leave room for any unexpected events that may cause delays. Please complete the enclosed forms and bring them by, mail them or fax them to us so we can enter your information into our computer system.
Bring a list of your current medications, a copy of your medical records, a photo ID, and your insurance card(s).
Please arrive 15 minutes before your scheduled appointment time in order to complete paperwork, as it is our policy to reschedule late arrivals to another day.
Payment is due at time of services. We accept all major PPO Insurances, Credit Cards, Checks and Cash.
If you cannot make your appointment, call us at least one day ahead so your appointment can be rescheduled.
